The hospital and the home are in the same sector, which is not true of most areas we cover. That is why most of our post discharge work in Gurgaon starts in these two sectors. A family can come out of the ward and be home without crossing the city, and the caregiver can be in the flat before the patient reaches it. Mayfield Garden, Ardee City and the Sector 52A residences hold most of the households. Park Hospital in Sector 47 is the third hospital families here use, mainly for orthopaedic follow up. Because of that hospital geography, the service mix in this zone is unlike the rest of Gurgaon. Post operative nursing is the largest category, then ICU step down care at home, then elderly attendants. ICU step down is the phrase families hear at discharge and rarely have explained, so here it is. It means the patient no longer needs intensive care but is not independent either. Oxygen may still be running. There may be a catheter, a feeding tube, a tracheostomy or a wound that needs dressing on a schedule. That is nursing work, not attendant work. It needs a qualified nurse who can read the discharge summary, follow it exactly, spot deterioration early and write observations a treating doctor can actually use at the follow up. So the first thing we ask for is the discharge summary, before we quote anything. It decides whether this is a GNM or ANM qualified nurse or a trained attendant, and quoting before reading it would mean sending the wrong person. On these cases we also ask which consultant is following up and when. The home routine should match what was written on the file, not run in parallel to it. Day to day, a step down placement is observations at set intervals, medication exactly on schedule, wound or catheter care as instructed, positioning to protect pressure areas, assisted feeding where needed, and a written chart at every shift change. An elderly attendant placement in Mayfield Garden or Ardee City looks different and calmer. Supervision, bathing, medication, meals, two walks, and a log in the evening. Ardee City is gated and low rise. Mayfield Garden is denser and busier. We ask which one you are in before allocating, because arrival timing and gate access are not the same in the two, and a nurse standing outside a barrier is a nurse not doing the job. Every caregiver carries police verification and an identity card, and nurses carry their GNM or ANM qualification. The family sees the record and approves the person before they travel from Kanhai. If the person is not right, we replace them. On a step down case we overlap the outgoing and incoming nurse for a shift so nothing about oxygen, tubes or medication timing is passed on verbally and lost. What changes the price here is clinical, not geographic. A qualified nurse costs more than a trained attendant, and on the step down cases that dominate these two sectors it is almost always the nurse. Twenty four hour cover costs more than twelve, and a step down discharge usually needs the full twenty four. Tracheostomy care, suction, oxygen management, catheter care and tube feeding all move a case up. A patient needing two people for every transfer is priced as two, and on a step down case that is common. A trial day exists but is rarely the right tool on a step down discharge, where the family needs the correct nurse on the first evening rather than an audition. Caregivers and nurses for this zone come from Kanhai, Wazirabad and Islampur. They work in Hindi and English, with Bengali and Malayalam on the roster. HUDA City Centre and the Sector 55 to 56 Rapid Metro are the nearest stations, and Artemis Hospital Road is the marker almost every family here uses for directions. Because the hospitals are inside the sector, a fair number of families here first speak to us in the ward rather than at home. If that is where you are reading this, the useful thing to send is a photograph of the discharge summary. It is faster than describing the condition, and it is what decides whether we send a nurse or an attendant. Readmission is common on step down cases and we plan for it. If the patient goes back into Artemis or CK Birla, the placement pauses rather than ends, and the same nurse returns when they come home. Families should not be re recruiting staff in the middle of a second admission. We also write down what we are not permitted to do. Our nurses do not change a prescription, adjust an oxygen rate or decide that a symptom can wait. Those go to the treating consultant, and the caregiver's job is to notice early and report accurately. Mayfield Garden, Ardee City and Sector 52A do not exhaust these two sectors.
