Post operative recovery, injections and dressings at home, physiotherapy, and support for diabetic and cardiac patients, delivered by trained staff working from a doctor's written instruction. We are specific about that because families in Kanpur are often offered untrained help under a general label. A person who can sit with a patient is not the same as a person who can dress a surgical wound, and the difference shows up in the third week. Many of these houses belong to business families whose sons and daughters now live in Delhi, Mumbai or abroad. For them the problem is distance rather than money. So the reporting is built for it. A written report at the end of every day covering what was eaten, what was taken, how the night went and anything that changed. A call each week at a time that suits the family rather than us. We put it in writing because a son in Mumbai cannot judge his father's condition from a two minute call in which the answer is always that everything is fine. Whether there is a wound needing dressing on a schedule, a catheter, a drain or a mobility restriction decides whether we send a qualified nurse or a trained attendant. Day to day, a post operative placement is dressing changes on the stated schedule with clean technique, medication exactly on time, and mobility only as far as the doctor cleared. It also means watching the wound for the signs of infection that appear before a fever does, and keeping a written observation log. Diabetic and cardiac support is the other steady category. Blood sugar checked and recorded at the set times, insulin given and stored correctly, meals genuinely matched to the diet rather than nominally, and feet checked for sores a patient with reduced sensation will not feel. These are old houses with deep steps, high thresholds and bathrooms built decades ago. On the first visit we walk the house and say plainly which surfaces are the risk, because a fall during recovery undoes the whole recovery. Every member of staff entering a Civil Lines house carries police verification, an identity card, and a record of qualification, experience and languages. Nurses carry their GNM or ANM qualification. The family sees that record and approves the person before they arrive. If the nurse is not right for the household, we replace them. That is a change of staff, not a renegotiation of the arrangement, and in a Civil Lines house we will say so to whoever abroad is arranging it. Twenty four hour cover costs more than a twelve hour shift, and many Civil Lines families use the shorter one with a night check. Injection and dressing visits are charged per visit rather than as a day. Physiotherapy is per visit. Night only cover is quoted on its own. A trial day is available first, and in Kanpur we usually suggest it, because most families here are arranging organised home nursing for the first time.
