Jalvayu Vihar in Sector 25 is a defence retiree society. Behind that single gate is a higher concentration of elderly residents than almost anywhere else we cover in old Noida.

Entry there runs through the RWA office rather than the individual flat. We clear at that level first, which makes day one slower and every day afterwards faster, and it means a caregiver is not being argued about at the gate each morning.

Sectors 26, 27 and 28 around it are older residences with much the same profile: long settled households, original allottees, and residents who have aged in the same flat.

Apollo is in Sector 26 and Kailash in Sector 27. Both are inside this zone's own sectors, which very few areas in Noida can say. Max Super Speciality in Sector 19 is a short run out.

That proximity is the reason post cardiac recovery is such a large part of what we do here. A family can be discharged and home without a long journey, and a caregiver can be in the flat with the medication schedule already written before the patient arrives.

Long term elderly attendant work is the biggest category, then dementia care, then post cardiac care, then physiotherapy.

Post cardiac work has a specific shape. Medication timing is not approximate. Fluid intake and weight are recorded daily because a sudden gain is the early warning of a problem.

Activity increases only as far as the cardiologist has cleared, and breathlessness or swelling gets reported the day it appears rather than at the next appointment.

Those are observations a trained person makes and writes down. A family member watching anxiously will notice the crisis but not the drift towards it.

Dementia placements run on sameness: the same caregiver, the same order of tasks, the same times, no rotation to cover a roster gap. In Jalvayu Vihar in particular, where a resident may have been in the same flat for thirty years, familiarity is most of the treatment.

Day to day, an attendant placement here is medication given and recorded, bathing supervised every time, meals to the prescribed diet, two walks around the block if the patient is steady, and a written log the family reads each evening.

Because these are long placements measured in years, we plan leave cover before the placement starts. The family meets the second caregiver early, while the patient is settled, so a handover day is never the first time that person walks in.

Every caregiver we send into Jalvayu Vihar or the surrounding sectors carries police verification, an identity card and a written record of experience.

In a defence society that record is also what the RWA office wants to see, so we file it there as well as with the family.

If the caregiver is not right, we replace them, and we re file the replacement's papers with the RWA rather than trying to walk someone in on the strength of the previous clearance.

Price follows condition and hours. A qualified nurse costs more than a trained attendant, and a post cardiac discharge from Apollo or Kailash usually needs the nurse.

Twenty four hour live in costs more than a twelve hour shift, and the long Jalvayu Vihar placements are usually the live in. Oxygen, catheter care, suction and tube feeding move a case to a nurse. Two person transfers are priced as two.

Physiotherapy is charged per visit.

A trial day is available before a long placement, though in Jalvayu Vihar the RWA step usually happens first.

Caregivers come from Nithari and Hoshiyarpur, the settlements immediately behind these sectors. They work in Hindi and English, with Punjabi, Bengali and Malayalam on the roster.

Punjabi is used heavily in Jalvayu Vihar and matters for patients who have reverted to a first language.

Noida City Centre, Sector 34 and Sector 18 are the nearest metro stations, and the Jalvayu Vihar and Sector 27 markets are the landmarks families use for directions.

Sectors 26, 27 and 28 are laid out around markets rather than towers, and the house numbering follows the old Noida pattern rather than a society block system. We take the sector, the house number and the nearest market together.

Jalvayu Vihar is the exception, with its own internal block system behind the gate.

One thing specific to a defence society is worth stating. Many residents here have entitlements through service, and hospital paperwork matters to them.

Our staff keep the daily record in a form that can be attached to a claim, rather than as loose notes that have to be reconstructed later.
