Most discharges do not need paid care at home. Some absolutely do, and those are the ones that come back through the emergency department a fortnight later. This page is about telling them apart before you spend anything.

## Pick care at home is needed when

- The discharge summary lists a medicine schedule nobody in the family has read yet.
- Getting to the bathroom is the thing everyone is quietly worried about.
- The family is going back to work on Monday and the patient will be alone.
- There is a dressing, a drain or a catheter that somebody has to watch.
- The patient has been readmitted before for the same condition.

## Pick family can manage when

- Somebody is home all day and is physically able to help.
- The patient is walking, eating and washing without help by the day of discharge.
- The prescription is short and unchanged from before the admission.
- The follow up is routine and somebody can take them.

## What to do either way

- Read the discharge summary out loud, with somebody else in the room. Half of what goes wrong in the first week is a timing that was misread.
- Write the medicine schedule on paper and stick it where it is seen. Not in a phone.
- Book the follow up before you leave the hospital, not later.
- Agree who is calling whom if something changes at 2am, before it is 2am.

## Questions

### Can care start the day of discharge?

Often yes, and in areas where the hospital is close it is routine. Message us while you are still waiting for the paperwork rather than after you get home, and we will tell you honestly whether we can reach you that evening.

### How long do families usually book for?

The first week or two is where readmissions come from, so most start there. You are not committing to a month. Book a visit, then a daily block if it is needed.

### Will the saathi change the dressing?

A saathi keeps it clean and dry, watches it and escalates. Changing a surgical dressing is clinical, so we coordinate a qualified nurse or the treating doctor for it.

### Can the saathi take them to the follow up?

Yes. They escort, wait through the appointment, and send you a written note of what the doctor said. If your loved one has to be taken there, we arrange the trip and charge what it cost, confirmed with you first.
